Formula: Residual toric effect = sin(2θ) × IOL cylinder power
Even 10° misalignment eliminates one-third of the cylinder correction. Intraoperative axis marking and alignment are therefore as important as the power calculation itself.
Clinical tip: Use a MICS slit-lamp marker or digital intraoperative guidance (e.g. Callisto/Verion) to confirm axis alignment within ±5° for best outcomes.

Astigmatism Classification
With-the-Rule (WTR) 60–120°Steep meridian vertical. More common in youth. PCA opposes → subtract 0.30D.
Against-the-Rule (ATR) 0–30° / 150–180°Steep meridian horizontal. More common after 60s. PCA compounds → add 0.30D.
Oblique Astigmatism 30–60° / 120–150°Steep meridian diagonal. No PCA adjustment applied.
Calculation Method
Vector Analysis (J0/J45)Double-angle power vectors used for all arithmetic. Avoids 180° axis discontinuity errors.
Holladay Vertex CorrectionELP = ACD + 0.3 mm. Vertex factor vF = 1 / (1 − ELP × net.mag). Converts corneal to IOL plane cylinder.
PCA (Baylor/Koch)Systematic offset for posterior corneal contribution. Not measured by standard keratometry. −0.30D WTR, +0.30D ATR.
