Formula: Residual toric effect = sin(2θ) × IOL cylinder power

Even 10° misalignment eliminates one-third of the cylinder correction. Intraoperative axis marking and alignment are therefore as important as the power calculation itself.

Clinical tip: Use a MICS slit-lamp marker or digital intraoperative guidance (e.g. Callisto/Verion) to confirm axis alignment within ±5° for best outcomes.

θ = degrees of misalignment
At 45° the toric cylinder adds induced astigmatism perpendicular to intent.
